Thank you for taking the time to view our school website.

If you choose to enrol with us, please complete the following forms. 

• Mellons Bay School enrolment form
• Mellons Bay School out of zone additional enrolment form

Once the forms are completed, you will need to come into our school office between 8.30am and 3.30pm, Monday to Friday, and bring with you the following documentation:

 Your child's birth certificate or passport

• Your child's immunisation certificate (found in your Plunket book or provided by your doctor)
• Verification of your home address (electricity bill required)
• Email address – complete this on the enrolment form please as we send out some newsletters and notices via email.

We are required to sight original documents and will take photocopies to keep with your enrolment form.
